Profiles of recovery from alcohol use disorder at three years following treatment: can the definition of recovery be extended to include high functioning heavy drinkers?

Profils de récupération d'un trouble lié à la consommation d'alcool 3 ans après prise en charge : une analyse secondaire d’essai randomisé

La guérison d'un trouble lié à la consommation d'alcool est souvent définie de manière réductrice par l'abstinence d'alcool et l'amélioration globale du patient (santé mentale, fonctionnement social, emploi). La présente analyse secondaire des données de l’essai clinique randomisé MATCH a examiné la variabilité des résultats de récupération, définis par la consommation d'alcool, les problèmes liés à l'alcool et le fonctionnement psychosocial 3 ans après la prise en charge thérapeutique. Une analyse secondaire a porté sur les facteurs prédictifs de profils latents avant et après le traitement et à 1 et 3 ans après le traitement.

Au total, les 806 sujets du groupe de consultations externes du projet MATCH ont été inclus dans l’analyse. La récupération a été définie par des analyses de profils latents comprenant le fonctionnement psychosocial, la qualité de vie perçue, le chômage, la santé mentale, la consommation d'alcool et d'autres drogues (Form 90), ainsi que les conséquences liées à l'alcool, 3 ans après la prise en charge thérapeutique.

Globalement, 4 profils ont été identifiés (1) grands buveurs et globalement défaillants, (2) grands buveurs peu performants, (3) grands buveurs occasionnels très performants et (4) grands buveurs occasionnels peu performants. Peu de différences ont été retrouvées entre les indicateurs de performance des individus et les variables liées au traitement chez les grands buveurs occasionnels très performants, autres qu’une faible dépendance à l'alcool (OR = 0,94 ; IC95% = 0,90 - 0,98), moins de capacités d’adaptation après prise en charge (OR = 0,54 ; IC95% = 0,32 - 0,90) et autosuffisance plus faible après 3 ans d’abstinence post-traitement (OR = 0,37 ; IC95% = 0,28 - 0,49) et implication aux Alcooliques Anonymes (OR = 0,87 ; IC95% = 0,85 - 0,99). Les 2 profils ayant un haut niveau de performance ont montré les améliorations les plus importantes en termes de fonctionnement, alors que les profils de bas niveau ont présenté les améliorations les moins importantes. Egalement, les buveurs excessifs occasionnels très performants avaient un but plus élevé dans la vie que les profils peu performants ou défaillants.

Ainsi, certaines personnes consommant beaucoup d'alcool après une prise en charge pour alcoolisme peuvent fonctionner aussi bien que celles en abstinence régulière concernant le fonctionnement psychosocial, l'emploi, la qualité de vie perçue et la santé mentale.

Abstract

Background and Aims Recovery from alcohol use disorder (AUD) is often narrowly defined by abstinence from alcohol and improvements in functioning (e.g. mental health, social functioning, employment). This study used latent profile analysis to examine variability in recovery outcomes, defined by alcohol use, alcohol‐related problems and psychosocial functioning at 3 years following treatment. Secondary analysis investigated pre‐treatment, post‐treatment and 1‐ and 3‐year post‐treatment covariate predictors of the latent profiles.
Design Secondary analysis of data from a randomized clinical trial.
Setting United States.
Participants We used data from the out‐patient arm of Project MATCH (n = 806; 29.7% female, 22.2% non‐white).
Measurements Recovery was defined by latent profile analyses including measures of psychosocial functioning and life satisfaction (Psychosocial Functioning Inventory), unemployment and mental health (Addiction Severity Index), alcohol and other drug use (Form 90) and alcohol‐related consequences (Drinker Inventory of Consequences) 3 years following treatment. Mixture modeling was used to examine correlates of profiles.
Findings We identified four profiles at 3 years following treatment: (1) poor functioning frequent heavy drinkers, (2) poor functioning infrequent heavy drinkers, (3) high functioning occasional heavy drinkers and (4) high‐functioning infrequent non‐heavy drinkers. There were relatively few differences on indicators of functioning and treatment‐related variables between the high functioning infrequent non‐heavy drinkers and the high‐functioning occasional heavy drinkers, other than high‐functioning occasional heavy drinkers having lower alcohol dependence severity [odds ratio (OR) = 0.94, 95% confidence interval (CI) = 0.90, 0.98], fewer post‐treatment coping skills (OR = 0.54, 95% CI = 0.32, 0.90) and lower 3‐year post‐treatment abstinence self‐efficacy (OR = 0.37, 95% CI = 0.28, 0.49) and Alcoholics Anonymous (AA) involvement (OR = 0.87, 95% CI = 0.85, 0.99). The two high‐functioning profiles showed the greatest improvements in functioning from baseline through the 3‐year follow‐up, whereas the low‐functioning profiles showed the least amount of improvement. High‐functioning occasional heavy drinkers had higher purpose in life than the poor‐functioning profiles.
Conclusions Some individuals who engage in heavy drinking following treatment for alcohol use disorder may function as well as those who are mostly abstinent with respect to psychosocial functioning, employment, life satisfaction and mental health.
